Dear Parent/Guardians,
All students entering grade seven must have a physical examination performed by a qualified physician. Nebraska State Law directs each school to require evidence of a physical within six months prior to starting seventh grade.
If your student is considering participating in any middle school sport, the physical exam must be done after May 1st. Students may not participate in any sports practice or game until the physical form is completed and returned to school. The required 7th grade physical will be accepted as the sports physical if completed after May 1st.
Every student will need to have a booster immunization containing diphtheria and tetanus toxoids and an acellular pertussis (Tdap). This immunization is a state requirement and needs to be completed in order to start school in the fall. If there are medical or religious reasons for your student not to receive immunizations, there is a waiver form available to sign upon request.
Please email, fax, or bring a copy of the completed physical and updated immunization record from your doctor’s office, before or on the first day of school. Any form generated from your physician’s office is acceptable.
